The physicals are $20, with all proceeds going to area schools. Checks should be made out to FHN. A portion of the physical form must be completed and signed by a parent before the exam. If a parent does not accompany their student, a Consent for Medical Care form (available at your local FHN office or at the school office) must be signed and brought with the student.
